第一篇：請假條的格式

請假條的格式

由于某種原因你不能去上學，或不能參加某項活動，就應向老師請假。

怎樣寫請假條呢？

請假條一般應包括以下幾個部分：

（1）稱呼。要項格寫，并在后面加冒號。

（2）正文。要另起一行，空兩格。主要寫明請假的原因和請多長時間假。

（3）請假人姓名和日期。寫在正文的左下方。

樣文如下：

李老師：

我因要去參加少年宮的大合唱彩排，今天下午的班隊活動不能參加，特此請假。

宋教師：

我因昨日感冒發(fā)燒， 現(xiàn)在體溫仍達39 ℃，故今天不能來上學，請準假壹天為盼。

領導會在你的請假條上簽字的。一般你們單位肯定規(guī)定：多長(更多請搜索：Www.)時間由誰來準假，要找對人，還有請假期間的待遇問題要協(xié)商好。
